Spinal stenosis means narrowing around the spinal cord or nerve roots. It can occur in the neck, mid-back, or lower back, and the effects differ by level. Lumbar stenosis may be associated with leg heaviness, pain, or numbness during standing and walking that improves with sitting or bending forward. Cervical stenosis can affect hand coordination, balance, strength, or sensation. Because these symptoms overlap with vascular, neurologic, and joint problems, the diagnosis requires more than an imaging label.
The examination may include gait, balance, reflexes, strength, sensation, range of motion, and questions about walking tolerance and hand function. Clinicians compare these findings with MRI, CT, X-ray, or prior studies when appropriate. The amount of narrowing shown on a scan does not always predict symptom severity, so treatment should be connected to the patient’s actual limitations and neurologic findings. A careful history also helps distinguish a gradual degenerative pattern from a new problem after an injury.
Non-surgical management can include education, activity pacing, medication review, and physical therapy aimed at strength, mobility, balance, and safer function. Selected patients may discuss injections for symptom control, recognizing that an injection does not enlarge the spinal canal. A surgical consultation may be appropriate when function remains substantially limited, conservative care has not provided adequate relief, or there are concerning neurologic findings. The procedure considered depends on the location and cause of narrowing, spinal stability, health history, and goals.
New trouble walking, rapidly worsening weakness, loss of hand coordination, falls, or changes in bowel or bladder control should be reported promptly. Severe or sudden neurologic changes may require urgent evaluation rather than waiting for a routine visit. Patients preparing for an appointment can note how far they can walk or stand, whether sitting changes symptoms, which side is affected, and what prior therapy, medication, or procedures have changed. Those details help measure function over time.
